Proxy Service


This service enables patients to allow another person - known as Proxy - to access their information and to act on their behalf. The proxy can be a family member, doctor or caregiver.

As the proxy can only look up the patient's information after log-in, the proxy has to be registered as a Premium Member.

To sign up for this service, patients have to :

  • include a copy of the relevant document, eg birth certificate, marriage certificate or any other legal document for verification purposes (please follow instructions on the form)
  • mail the completed form and document to NetCare Internet Services, 6 Commonwealth Lane #05-01/02 GMTI Building S(149547)
